To help you get acquainted with our office and our policies, please read our Office Policy and New Patient Package Forms below. Please take a few moments prior to your appointment to review and complete these registration forms. We ask that you bring the completed forms with you to your appointment, along with your insurance card.

Appointments

We request that appointments be made in advance. Please make sure you bring your photo ID, insurance card, and a bag with all of your prescription medications, herbal supplements, and over-the-counter medications. Please arrive at least 15 minutes before your scheduled appointment time. If you are a new patient, please bring your medical/immunization records from your previous provider. If you are in need of immediate care, please walk-in or call for an appointment.

No Show

If you fail to show three times for your scheduled appointment, you may lose the privilege of scheduling your appointment in the future and will have to walk into the office and wait to be seen by a provider.

Office Hours

Standard Chesapeake Health Care office hours are from 8:30 am to 5:00 pm. However, certain offices have extended hours to suit your needs. Please visit our locations page to view the individual office hours and updated telephone numbers.

If you are unable to keep your appointment, please give us 24 hours notice. This allows us to schedule other patients. Also, please report any changes in name, address, phone or insurance information when you phone or visit the health center.
