School-Based Special Programs

Expanding Access to Behavioral Health Services for Students and Families

Chesapeake Health Care is dedicated to enhancing access to comprehensive behavioral health services for students and families across Wicomico and Worcester County.

Through partnerships with the Community Health Resources Commission (CHRC), Chesapeake Health Care is working to improve student well-being and academic readiness. These grant-funded initiatives aim to broaden access to a range of services, including:

  • Preventative programming
  • Universal supports
  • Individual, group, and family therapy
  • Wraparound, case management, and care coordination services
  • Substance use treatment
  • Behavioral health education, support, and navigation for families
  • Crisis planning and services
  • Telehealth services
  • Support groups, and more

Our goal is to ensure these vital services are easily accessible to students and families. Each school’s referral process may vary and typically involves school support staff. This initiative empowers school personnel to connect students and families with community providers to address their holistic needs.
