Chesapeake Health Care participates with most insurance plans, and will bill those insurance plans accordingly. Chesapeake Health Care will bill you for any remaining balance if indicated. It is important for you to understand the terms of your insurance coverage. If the plan requirements are not followed, you could be held financially responsible for all or part of the services received at Chesapeake Health Care.

If you have any questions regarding your bill, please call the Billing Department at 410-749-1015. You may use your MasterCard, Visa, Discover, or American Express card to pay your portion of the bill. It is our goal to assist all of our patients to find payment options as appropriate. Please be aware that we reserve the right to collect unpaid bills.

If You Do Not Have Insurance – Our Certified Application Counselors will assist you in applying for Medical Assistance, Qualified Health Plans, or Sliding Fee.

Payment Policy

Payment is expected at the time of your visit. This includes co-payments required by your insurance company. We offer a Sliding Fee Program to ensure that our services are available to everyone, regardless of ability to pay. Please inquire about the Sliding Fee Program or payment arrangements if you need more information.

We accept cash, personal checks, credit and debit cards from MasterCard, Visa, Discover, or American Express, as well as payment from most insurance companies. A fee may be charged for any checks returned for non-sufficient funds.

Non-Payment

It is our goal to assist all of our patients to find payment options as appropriate. Please be aware, however, that we reserve the right to collect unpaid bills.

Insurance

We accept most major insurance companies and managed care health plans, including but not limited to Aetna, CareFirst Blue Cross Blue Shield, Integra, Conifer, Tricare Prime, United Healthcare, Cigna, as well as Medicare and Medicaid. We participate with Medicaid Managed Care plans including Priority Partners, Amerigroup, University of Maryland Health Partners, and United Healthcare Community Plan. We recommend that you check with your managed care plan to ensure that the health center is named as a participating provider. If your plan requires you to choose a primary care provider (PCP), be sure to name your health center provider. Your insurance card contains the correct information we need to bill your insurance for you—please bring your card to each visit.

Maryland HealthChoice

Maryland HealthChoice is the name of Maryland’s statewide mandatory managed care program. The HealthChoice Program provides health care to most Medicaid participants. Eligible Medicaid participants enroll in a Managed Care Organization (MCO) of their choice and select a primary care provider (PCP) to oversee their medical care.

Sliding Fee Program

As a Community Health Center, we offer a Sliding Fee Program based on household income and family size, which reduces the amount you pay for healthcare services. If you qualify, you may pay only 20 – 80% of the cost for most services. You may be eligible for this program even if you have insurance. Applications are available at the reception desk, at each site. All information on the application is kept confidential. We have designated staff available to help you with completing these applications.

Federal Poverty Guidelines will be used for the Sliding Fee Program. If your income falls within the guidelines, we encourage you to apply. Please see our Sliding Fee Program page for more information.

Proof of income is required to process your application. A copy of your most recent Federal Income Tax Return is strongly encouraged as proof of income. However, the documents listed below are also acceptable:

  • Workers’ Compensation
  • W-2 forms
  • Income tax returns
  • Social Security notice
  • Pension notice
  • College Students may use parents’ income
  • Current pay stubs
  • Unemployment award notice
  • Letter from employer
