Behavioral Health Integration Specialist – Salisbury MD

The Behavioral Health Integration Specialist is responsible for assessing interested patients, designing care coordination plans and coordinating care management for CHC patients. The Behavioral Health Integrated Specialist is based primarily in the primary care clinic, and provides screening, assessment, evaluation of needs, care management plan development and linkage to higher levels of care for patients interested in recovery support for substance use disorder. Works with a multidisciplinary care management team. This position reports to the Mental Health Operations Manager/Clinical Team Leader. This is a Monday through Friday position working 40 hours per week. Full benefit package.

Essential Job Functions of this position include but not limited to:

  • Conducts brief, evidence-based and developmentally appropriate screenings and arranges for more detailed assessments when indicated.
  • Functions effectively as a member of an inter-professional team that includes behavioral health, primary care providers, dieticians, pharmacists, consumers, and family members.
  • Establishes rapport quickly and communicates effectively with consumers, family members, and other providers using active listening in a non-judgmental manner.
  • Use focused interventions to engage consumers and increase their desire to improve health
  • Provide health promotion, wellness, and prevention interventions.
  • Provides emergency assessment of patients in crisis and facilitates psychiatric hospitalization if needed.
  • Routinely conducts brief screenings regarding self-harm, harm to others, impairments in functional self-care, and environmental safety.

  • Recognize and diagnose, using established classification criteria, mental health and substance use conditions.
  • Create and periodically update integrated care plans in consultation with health consumers, family members, and providers.
  • Through the care plans link multiple services, health care providers and community resources to meet the health care consumer’s needs.
  • Provides a range of brief focused prevention, intervention and recovery services using motivational interventions, health promotion and wellness services, health education, and crisis intervention.
  • Provides referrals for internal and external services for those consumers with persistent illness that require follow up over time.
  • Provides consultation and treatment recommendations to primary care providers in the areas of behavioral health and substance abuse issues.
  • Verbal, nonverbal, and written communications are informative and well-integrated.
  • Use electronic health care record to retrieve relevant information and document relevant information concisely.

Qualifications

Education

  • Bachelor’s degree in social work or other related human service field.

Experience

  • Minimum three years casework/case management or related experience.
  • Capacity to work with individuals in a confidential, respectful, caring, professional manner.
  • Strong communication skills both verbal and written, including experience in motivational interviewing.
  • Adequate computer skills required (MS Office, web based software, and some knowledge of electronic health records system).
  • Must have strong interpersonal skills.
  • Excellent organizational skills needed.
  • Must be able to work independently and as part of a team.
  • Drug and Alcohol counseling certification/experience preferred but not necessary
  • Experience in working with mental health, addiction, recovery, and substance use disorder strongly preferred.

Dental Assistant – Princess Anne, MD

The Pharmacy Technician fills prescribed medication for patients by selecting the proper medication, counting the tablets, labeling the bottles.  Technicians set up and maintain patient profiles, call insurance companies, stock shelves and take inventory of prescription and over the counter medications. This position is a 40 hour per week position. Shifts range from 7:30am to 6:00pm Monday through Friday. An occasional Saturday of 9:00am to 4:00pm will be required.

Position

Essential Job Functions of this position include but not limited to:

  • Collaborates with providers and practice staff in identifying appropriate patients for care management, utilizing established Care Management criteria.
  • Performs initial and periodic holistic assessments for care-managed population. This includes physical and psychological assessments as appropriate. The assessment includes a systematic and pertinent collection of data about the health status of the patient. Prioritize patients according to intensity, need and required follow up.
  • Formulates and implements a care management plan that addresses the patients identified needs by assessing the patient/family needs, issues, resources and care goals; determining the choices available to individual patients; educating the patient/family on the choices available.
  • Establishes a care management plan that is mutually agreed upon by the health care team and the patient/family. Plans will contain specific mutual self-management goals, objectives, and interventions with the patients are action-oriented.

If certified in Advanced Practice with prescriptive authority, may make medication adjustments as needed in collaboration with Primary Care Provider.   •  Collaborates with the healthcare team to revise the care management plan when changes occur. Initiates care conferences to discuss multidisciplinary team responsibilities, patient progress, new problems, etc.
  • Identifies and effectively utilizes community resources to meet the needs of patients/families. Facilitates patient access to community resources as appropriate or refers to LSW.
  •  Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence. Interacts professionally with patient/family and involves patient/family in the formation of plan of care.
