Chesapeake Health Care’s Adult and Family Medicine Department focuses on family, adult, and geriatric patients. We provide comprehensive and coordinated primary care. We serve a culturally and socioeconomically diverse patient population in the Lower Eastern Shore area of Maryland. Our Adult Medicine clinicians provide expert coordinated primary care for patients over 18 years of age.

We now offer telemedicine appointments. Skip the trip and securely connect with your CHC providers from the comfort of your home. Call your office of care to make an appointment.

We provide adult and family primary care services with a team of professionals, including your Primary Care Physician, Physician Assistant or Nurse Practitioner (NP). We provide individualized outpatient medical care with a team of healthcare professionals to provide a multidisciplinary approach to patient care.

At Chesapeake Health Care, we place a strong emphasis on preventive care, health education, and appropriate disease management, and medical interventions. Your relationship with your Primary Care Doctor, Physician Assistant or Nurse Practitioner is designed to be long-term and ongoing so that we can provide continuity of medical care, and allow you to make informed decisions about your health care needs.
