Our History

Three Lower Counties Community Services, Inc. was incorporated in 1994 to provide primary medical and dental care to patients in Somerset, Wicomico, and Worcester counties in Maryland. Since then, our services have expanded to include care for migrant/seasonal farm workers, mental health, pharmacy, obstetrics and gynecology, urogynecology, and pediatrics.

In 2007, we merged with Eastern Shore OB/GYN, Drs. Travitz and Crick, Dr. Cooper and Mr. Jerry Clayville, in order to further enhance health care access for patients. The Dental office in Princess Anne, Maryland opened a 24-chair facility in 2010. The Adult Medicine office at Woodbrooke, Salisbury, Maryland opened in 2010, as well as Delmarva Pharmacy in the same complex.

In 2016, we changed our name from Three Lower Counties Community Services to Chesapeake Health Care to more accurately reflect our core mission, which is to provide affordable, culturally competent patient and family-centered health care. As a Community Health Center we serve the healthcare needs of the three lower counties of the Eastern Shore of Maryland.

We recently opened a new OB/GYN facility, which will be located on the Woodbrooke Medical Campus in Salisbury. This facility will replace the OB location at the Riverside Medical Complex and the GYN location on Phillip Morris Drive.This brand-new 26,700 sq. ft. facility will have 32 exam rooms, 7 obstetricians/gynecologists and 3 gynecologists.

Chesapeake Health Care has been accredited by the Joint Commission since 2002, and is certified as a Patient Centered Medical Home. As a Federally Qualified Health Center, we provide the highest quality, affordable, culturally competent health care and preventive service. We offer a full range of pharmacy services as well.

Our employee workforce has grown to more than 400 people. In 2015, more than 45,000 patients received health care services. We continue to grow and provide exemplary service to our patients.

Chesapeake Health Care is a Joint Commission-accredited ambulatory health care organization. CHC underwent a rigorous onsite survey on November 8-11, 2016. During the review, a team of Joint Commission expert surveyors with expertise in ambulatory health care, evaluated compliance with standards related to how effectively the primary care clinician and the interdisciplinary team work in partnership with patients to provide comprehensive, coordinated and patient-centered care.

CHC has also earned The Joint Commission’s Gold Seal of Approval® for Primary Care Medical Home (PCMH) Certification. The Gold Seal of Approval® is a symbol of quality that reflects an organization’s commitment to meeting optional standards demonstrating a commitment to patient-centered care. PCMH Certification is a special add-on option for Joint Commission-accredited ambulatory health care organizations.
Achieving Joint Commission status means that Chesapeake Health Care has been recognized for complying with rigorous national performance standards that promote quality healthcare delivery.
