Pfizer Vaccine Clinic for Ages 12+

Pfizer vaccine will be available at this clinic for patients ages 12+. When registering, please put the child’s information in the appointment slot(s) below.

Parent/Guardian MUST be present to sign consent and for any child under the age of 18 to receive the vaccine. It is very important to keep the appointment to prevent wasted doses.
